Injuries on Playground Equipment?

Parents, have any of you had children who have obtained a serious injury on playground equipment? If you could, please share your experience. When, how, where, how old was your child and what happened to him/her? What do you feel you could have done differently? I am trying to write a paper about the dangers of playground equipment, specifically for children under 5 years of age. Thanks!
